What situation have I use small or medium?

I have medium T4, my accumulator goes to 0 so fast...

in any case it being a bot and not a Mech immediately says you should not be using medium gear.  Check the fitting reqs esp accu usage and look at your bots recharge time and accu cap that should tell you not to fit and run it.

Even if it is a mech (Tyrannos), a medium repairer will still drain your accumulator fairly quickly.  Those arent meant to be run permanently, but for faster boosts of more HP at the cost of your accumulator.  If you want something thats more efficient and able to be run indefinitely, run a small repairer on the Tyrannos.  The small repairers are the most efficient (give most HP per AP used) so you use less accumulator to repair damages.
