My friend saw this on the steam sale and asked me about it ... I remember playing this in Nov 2010 when it first came out - so I went to redownload it now that its free to play but under account management it says I need to buy the game?

The game was given to any player that subscribed 3 or more months during the old subscription model. If you subscribed for less than 3 months then you would need to purchase a copy of the game for those accounts.
In the past Perpetuum was subscription-based. In April 2014 we have changed that to a "buy once, play forever" model.
When the change happened, old players who had at least 3 months of accumulated subscription time received the game for free. Looks like your account had only 1 month, so it's currently inactive and you would need to buy the base game to play, yes.
